The Independent Police Complaints Commission (IPCC) has concluded its investigation into the actions of an officer who changed a note on his computer before giving evidence at the inquest of Jean Charles de Menezes. The investigation found that the officer acted naively but there was no evidence of deliberate deception.

The IPCC has today advised lawyers acting for Jean Charles de Menezes' family, the Metropolitan Police Service and the Metropolitan Police Authority that it will stand by its decision not to recommend disciplinary actions against the MPS officers involved in the fatal shooting of Mr de Menezes in July 2005.

The IPCC has received a referral from the Metropolitan Police Service in relation to a matter arising from the inquest into the death of Jean Charles de Menezes.
This matter will be subject to an independent investigation and we are not in a position to comment further at this stage.

The Independent Police Complaints Commission has agreed with the Metropolitan Police Authority (MPA) that Assistant Commissioner Andy Hayman should receive 'advice' as a result of the 'Stockwell Two' investigation.

The Independent Police Complaints Commission (IPCC) made 16 recommendations for change to the police service as a result of the Stockwell investigation. These recommendations were addressed to HMIC, Home Office, ACPO and the MPS.
In drawing up these recommendations the IPCC has worked closely with Her Majesty’s Chief Inspector of Constabulary (HMIC.) The recommendations were:
